Important Notices

  • It your utilities are shut off or subject to shut off, call your local HEAT office immediately, and do not submit a mail-in application. 
  • Submittal of HEAT application does not guarantee approval. 
  • Applications may take up to six weeks to process; therefore, it is important to continue to pay your utility bills. 
  • Sign and date the application.  Keep the pink copy for your records
  • Use appropriate postage, and mail to the correct address for your county listed on the link below.
  • Respond promptly to requests to provide missing or additional documentation (see list below).  
  • Failure to provide necessary documentation will result in denial. 
  • You will be notified by mail when your application is approved and what the benefit amount will be.
  • Denial notifications will also be mailed with an explanation of reason for denial. 
  • If you mail in an application, you do not need to schedule an appointment.

NECESSARY DOCUMENTATION:

Household Verifications

  • Copy of your picture identification.
  • Copies of all Social Security cards for everyone in your home who is 18 years and older.
  • Copies of US Citizenship & Immigration Services (USCIS) documents showing legal U.S. residency. Acceptable USCIS documents.
Income and Income Deductions Verifications
    • Copies of the proof(s) of income received in the previous month by all household members.  (Check stubs, Social Security, Retirement, Child Support, Alimony, etc.)
    • Self employment 1040 tax record or complete this form Self Employment Worksheet
    • If anyone in your household is a college student receiving financial aid, include a copy of each one you are receiving. Some examples of financial aid are:loans, grants, scholarships, stipends, assistantships, fellowships, veterans’ benefits for schooling, workstudy any other types of school financial aid. (most types are not counted as income).
    • If you are claiming zero income, you must complete this form: Zero Income Form
    • Copies of proofs of any medical expenses that were paid in the previous month; including health insurance premiums, co-pays, receipts for paid medicine, eyeglasses, dental, and over-the-counter medications. 
    • Proof of any child support or alimony paid the previous month, if applicable. Alimony Child Support Form

Energy Burden Verifications

    • Copies of your most recent Gas and Electric utility bills.  (If for some reason these bills are not in your name, tell us who the person on the bill is and why it is in their name.)
    • If you do not pay for energy costs directly, please provide a landlord’s statement or other verification stating that part of your rent goes toward paying utilities. Landlord Statement Form
  • Target Groups Verifications:  Additional money can be given if you have a person over 60 years old, anyone is disabled, or a child five and under.
    • Copy of the birth certificate for a child five years old or younger, if in the home. 
    • Proof of a disability, if applicable.  Medical Disability Form
ADDITIONAL DOCUMENTATION MAY BE REQUIRED.
