You can't go on to the applications until you first read the following instructions.  The application links will then be at the bottom of this page.  You want your application to get approved don't you?  Well, in order for your application to be considered for approval, you must meet the eligibility requirements of the HEAT program (see Eligibility on the left) and follow the instructions below.

Additional money can be given if you have a person over 60 years old, anyone who is disabled, or a child five and under living in your household.

There are 2 types of HEAT applications:  Choose the one that suits your needs best.  

Interactive HEAT Application You can type directly on this application, (helps us read it better) and it calculates income and deductions. After completing the application, print it, make sure you sign and date it and KEEP A COPY for your records.  Then mail the application and all related documents to the HEAT office in the county where you live. 

2011-2012 HEAT Application (Printable) You can print this off, fill it out by hand, make sure you sign and date it and KEEP A COPY for your records.  Then mail the application and all related documents to the HEAT office in the county where you live.
